Environmental Health Community Survey 2017

The Florida Department of Health in Clay County needs the help of residents and partners to understand environmental health in Clay County. We need YOUR assistance to distribute this survey throughout the community. Please share through your channels and contacts (and don’t forget to take the survey yourself!). This survey will help us collect opinions about the environment and quality of life in Clay County. Results will be made available to the public in a written report. The information gathered from responses to this survey will help make Clay County a better place to live. Thank you, in advance, for your participation! The survey will close November 30, 2017 at midnight.
The survey can be completed electronically by downloading and completing the pdf version of the survey and submit to Caitlin Murphy at caitlin_murphy@hpcnef.org.
