The Florida Department of Health in Clay County (DOH-Clay) invites Clay County residents to attend the annual Community Health Improvement Plan (CHIP) Meeting on Tuesday, May 12, 2026.

Clay County’s CHIP is a collaborative effort that brings together different people and groups to improve the health of the community. Our goals, shaped by the Community Health Assessment, focus on important health issues over a five-year period. The community is invited to attend as we review progress from the past year and hear updates from community partners about key health topics that impact residents.
