Death Certificates

Florida death records from 2009 to present are available at any Vital Statistics office in Florida.

All other Florida deaths must be obtained from the state office in Jacksonville. Anyone may obtain a certified copy of a death certificate, but only the immediate family may obtain a certified copy with the cause of death.

Valid photo ID (front and back) is required when requesting the cause of death. The fee is $10 per copy. You may download an application and apply in person or by mail.

The Vital Statistics Office is open Monday through Friday from 8 a.m. to 5 p.m. You may also order online through VitalChek, our ONLY authorized online ordering vendor.

Death certificates are available in Fleming Island at 1845 Town Center Blvd., Building 400. The tax collector office does not issue death certificates.
