In anticipation of the new school year, DOH-Clay will be providing immunization services Monday through Friday, Aug. 3 to Aug. 17 from 8:00 a.m. to 2:00 p.m., at the Clay County Fairgrounds, 2497 State Road 16 (Building 3), Green Cove Springs, FL 32043.
Immunizations are offered at no cost; however, there will be a $12 fee for any child receiving a DH680 or DH681 form without vaccine services.
