If you require in-person services from our Enviromental Health team (septic/well permits), Call 904-529-3784 to make a walk-in appointment at least 3 hours prior to arrival of 477 Houston Street, 3rd Floor.
Vital Statistics
The Vital Statistics Lobby is open
- We encourage all customers to wear a mask (face covering).
- If possible, please complete an application for the record you need prior to arrival. Applications can be printed through our website. Please bring a pen.
Orders can be placed online through www.vitalchek.com and delivered via USPS or overnight delivery.
