How To Submit Immunization Records
Below is the process to submit immunization records for transfer. A current working phone number is required to be entered on the immunization records transfer form or transfer will not be processed.
Submit documentation by one of the following options:
Option 1: Visit our online portal and complete all required fields.
Option 2: Drop documents off in person at the Florida Department of Health in Clay County (DOH-Clay).
If choosing this option, please drop off records 48 hours prior to your scheduled appointment.
- Keep copies for your records; never submit original documents. Make sure writing is legible.
- All records will be processed within 48 hours of receipt. A nurse will notify parent/guardian when record is entered into FLShots and advise parent/guardian if child will need additional vaccines. Illegible/incomplete patient information will be be processed.
- Copies of immunization records can be picked up in person at DOH-Clay. Records cannot be faxed or emailed.
