APPOINTMENT REQUIRED FOR ALL IMMUNIZATION SERVICES

IMMUNIZATION RECORDS ARE REQUIRED WHEN SEEKING IMMUNIZATION SERVICES

Unless all immunizations have been recorded in Florida SHOTS, parents/legal guardians are to bring immunization records with them. Not all Florida providers record immunizations in Florida SHOTSThe clinic does not request immunization records on behalf of parents/legal guardians. When moving from out-of-state, it is strongly encouraged that parents/legal guardians obtain a paper copy of all immunizations prior to moving to Florida.

In addition, it is recommended that international vaccine records be dropped off 48 hours prior to appointment to allow for translation.

How To Submit Immunization Records

Below is the process to submit immunization records for transfer. A current working phone number is required to be entered on the immunization records transfer form or transfer will not be processed.

Submit documentation by one of the following options:

Option 1: Visit our online portal and complete all required fields.

Option 2: Drop documents off in person at the Florida Department of Health in Clay County (DOH-Clay).

If choosing this option, please drop off records 48 hours prior to your scheduled appointment.

  • Keep copies for your records; never submit original documents. Make sure writing is legible.
  • All records will be processed within 48 hours of receipt. A nurse will notify parent/guardian when record is entered into FLShots and advise parent/guardian if child will need additional vaccines. Illegible/incomplete patient information will be be processed.
  • Copies of immunization records can be picked up in person at DOH-Clay. Records cannot be faxed or emailed.

Children under 18 years of age need to be accompanied by a natural or adoptive parent, legal custodian, legal guardian, or person who possesses a power of attorney to provide medical consent for the minor. A step-parent, grandparent, an adult sibling of the minor, an aunt, or an uncle can bring the minor child in for immunizations, but they MUST have notarized written consent from a natural or adoptive parent, legal custodian, legal guardian, or person who possesses a power of attorney to provide medical consent for the minor.

Every child entering a Florida school for the first time needs the Florida Certification of Immunization form (also known as Form 680). This form must be completed by a physician or a Florida county health department.

Religious Exemption

Religious exemption from immunization is issued if immunizations are in conflict with the religious tenets and practices of the child's parent or guardian. This exemption is issued by the Florida Department of Health in Clay County -  Clinic (1845 Town Center Blvd., Building 400, Fleming Island, FL 32003) Monday – Friday,  8 a.m. - 11 a.m. and 1 - 4 p.m.  The religious exemption from immunization is based on established religious beliefs or practices only.

Limited adult immunizations are available. These immunizations are provided at our cost, plus a $15.00 injection fee.
