What To Bring 

  • Copy of most current immunization records in English*
  • Photo ID
  • Insurance Card

IMMUNIZATION RECORDS ARE REQUIERED WHEN SEEKING IMMUNIZATION SERVICES

Unless all immunizations have been recorded in Florida SHOTS, parents/legal guardians are to bring immunization records with them.  Note: Not all Florida providers record immunizations in Florida SHOTS.  The clinic does not request immunization records on behalf of parents/legal guardians. When moving from out-of-state, it is strongly encouraged that parents/legal guardians obtain a paper copy of all immunizations prior to moving to Florida. 

*If immunization records are not in English, contact the TRAVEL CLINIC JACKSONVILLE at (904) 722-8747 to make an appointment for record translation to English.

Who Can Bring Children for Immunizations 

Children under 18 years of age need to be accompanied by a natural or adoptive parent, legal custodian, legal guardian or person who possesses a power of attorney to provide medical consent for the minor.  A step-parent, grandparent, an adult sibling of the minor, an aunt or an uncle can bring the minor child in for immunizations if they have written consent from a natural or adoptive parent, legal custodian, legal guardian or person who possesses a power of attorney to provide medical consent for the minor and we can reach that person by phone.

Back to School Rush - Immunization Clinic (7/30/18 thru 8/24/18)

During times of high patient volume including the Back to School Rush – Immunization Clinic, clients may experience longer lines and wait times due to the high volume of people being served. We encourage everyone to not wait until just prior to school starting.

Religious exemption

Religious exemption from immunization, is issued if immunizations are in conflict with the religious tenets and practices of the child's parent or guardian. This exemption is issued by at the Florida Department of Health in Clay - Bear Run Clinic (3229 Bear Run Blvd. Building B Orange Park, Florida 32065) Monday – Friday 8 am – 11am and 1 pm – 4 pm.  The religious exemption from immunization is based on established religious beliefs or practices only.

Adult Immunizations 

Limited adult immunizations are available. These immunizations are provided at our cost plus a $10.00 injection fee.

  • Tetanus (TDAP)
  • Hepatitis B
  • Measles/Mumps/Rubella (MMR)
  • Pneumonia
  • Menactra
  • HPV

Notice: Most private insurance does not cover immunizations. Contact the clinic to obtain pricing and availability.
