Are you struggling to afford insulin and diabetes supplies?

The Florida Department of Health in Clay County offers no-cost and reduced-cost insulin and supplies to individuals who qualify.

Eligibility Requirements

  • Self-declared resident of Florida
  • Diagnosis of diabetes
  • Uninsured, lacking insurance that covers insulin or diabetes-related supplies, or has an insurance deductible or copay that the applicant cannot afford
  • Net family income at or below 100% of the federal poverty guidelines
  • Has no more than $2,500 in private funds, bank accounts, or assets, other than a homestead
  • Not a current Medicaid recipient

Program Application

Individuals applying to participate in the program must complete the program application and provide a current prescription for insulin.

Medications and Supplies

  • Insulin: Humalog, Lantus, Humulin N, Humulin R, and Humulin 70/30
  • Diabetes-related supplies: Glucagon (Baqsimi), glucometer, test strips, lancets, and insulin syringes
