This program is part of a statewide oral health plan to promote evidence-based prevention strategies in order to achieve optimal oral health for all Floridians.
Through a partnership with DOH-Baker and the Clay County School Board, DOH-Clay participates in the School-Based Sealant Program at many Clay County schools.
Consent Forms
Parents and guardians will need to complete a consent form packet for each student participating in the program. You can complete the online form or submit a paper form to your child’s teacher or school nurse.
